Individuals with ABI/TBI, who qualify through our assessment process are matched with a Client Service Coordinator (CSC). Once a person is accepted into the program, a community health assessment (CHA) will be completed at intake and then annually. The CSC assists the individual to address their priorities and to develop a care plan. The plan is then implemented, reviewed and assessed on a regular basis.
Individuals with ABI are then assisted with skills development in planning, organizing, and completing necessary documentation for services such as:
- Health Links
- Coordinated Care planning
- ODSP, CPP, OW, supportive and RGI Housing
As well, assistance is provided with:
- accessing links to health care services
- following up and collaborating with primary care practitioners and other health care providers
- connecting to community resources
- securing housing and food security
