If you, a loved one, or a clinical client requires immediate intake support, the entry point begins with our intake office. No medical referral is strictly required to start an initial discovery query:

BIAPR offers non-medical ABI community support services. An application can be made by the individual (Self-referral) or Professionals (primary health care/community partners). To access services from BIAPR please click on the BIAPR Application for Service button for a printable PDF form. BIAPR operates under the PHIPA guidelines. We welcome applications via Fax, Mail or In Person. For reasons of confidentiality please do not email Application.
