Co-ordinated Care Café is BIAPR’s newest service. It supports individuals who are living with complex care challenges that include ABI, Mental Health and/or Addiction. With the support of Community Case Management, Case Worker and a specialized ABI Day Service, BIAPR delivers wrap-around services to clients with additional needs.
The community support services are adapted to the individual client’s needs in a facilitated ABI community setting. This approach provides the tra opportunity for community collaboration with social services, primary health care, justice, and other community supports, to prevent vulnerable clients from falling through cracks in the system.
This new service is funded by the Central East Local Health Integration Network (CE LHIN).
