Community Support & Case Management

Following acceptance through our intake track, individuals are formally matched with a dedicated Client Service Coordinator (CSC). We implement an intensive Community Health Assessment (CHA) to build a realistic, protective care plan. The CSC guides the client through practical skill development, including:

  • Organizing, planning, and filing complex documentation for critical assistance services (including ODSP, CPP Disability, Ontario Works, and Rent-Geared-to-Income supportive housing frameworks).
  • Directly collaborating with primary physicians, psychiatrists, and specialized therapists to unify the client’s care loop.
  • Securing stable housing assets, managing local food security connections, and setting up daily independent living goals.
